As you may know, on April 1st, 2006, Wescreen was aquired by Tracers Information Specialists, Inc.

To comply with our current compliance procedures, all Wescreen clients must return all required applications for services by Thursday, October 5th, 2006 to remain active. All accounts not in compliance will have access to the system suspended until all necessary paperwork has been received.  Please visit www.wescreen.com and click on subscribe to download the applications or you may call our office at 877-723-2689 to have the applications faxed or emailed to you.

Application for service, FCRA Compliancy, Affidavit of Intended Use

As of October 2nd, 2006, some driving records will now require a signed release form. The following states will be affected.  Please fax completed release forms to 877-723-2691. New Hampshire release forms must be notarized.  Release forms are available by clicking the links below.

  • Colorado Release Form
  • District of Columbia Release Form
  • Georgia Release Form
  • Illinois Release Form
  • New Hampshire Release Form

    As of October 2nd, 2006, all signed release forms required for credit reports, driving records (if applicable,) education verifications & worker's comp (if applicable) must be faxed to 877-723-2691.  These reports cannot be processed without a signed release form.

    Credit Reports: All orders must be accompanied by a signed release form.

    Driving Record States requiring a signed release: Alaska, Arkansas, Colorado, District of Columbia, Georgia, Illinois & New Hampshire. (New Hampshire must be notarized.)

    Workers Comp States requiring a signed release: Alabama, Colorado, Connecticut, Idaho, Kansas, Missouri & Washington.

    Tracers Information Specialists, Inc., founded in 1996, has become the leader for our compliance standards within the industry. Therefore, in a continuing effort to ensure that our compliance procedures are second to none, we are requiring all of our existing customers to fill out our application for services. You may download a copy by clicking on subscribe on Wescreen.com or please call our office at 877-723-2689 and a representative will gladly fax you a copy.

    The application for services will need to be completed and returned to us no later than September 1st, 2006 in order to continue services with us. Unfortunately, failure to comply with this requirement will result in temporary suspension of your service. Your prompt attention will not only ensure that you will not experience an interruption to your service but will also allow us to continue providing you with the excellent data you have come to expect.
